Muskoka Health Link Help Patients From Falling Through The Cracks
Last year, the Muskoka Health Link Team helped keep hundreds of Muskokans from falling through the cracks of the healthcare system.
That was the message during a recent meeting of the District of Muskoka’s Health Services Committee. Muskoka Health Link collaborates with health and social services providers in
Muskoka developing coordinated care plans for patients with complex needs. The committee heard that in 2019, the team helped some 210 Muskoka residents.
Many people fall through the cracks as they move between different areas of the health and social services system. According to their report, many Muskokans are forced to navigate the healthcare system on their own, with limited information. They can face barriers or become lost in the system as they try to move between the different areas of care, the committee heard.
Five percent of people in Ontario account for two-thirds of health care costs. These are most often individuals with multiple, complex conditions.
The committee heard that Muskoka Health Link clients were high users of the Emergency Department, with 71% of the attendees having made at least one visit within the past year. Just under one-quarter of clients had more than three organizations involved with their health care needs, and 82% of clients were seniors.
